CareFirst BlueCross BlueShield said its Patient-Centered Medical Home (PCMH) costs were $314 million less than expected in 2014, demonstrating that the “program continues to show dramatic impacts on overall medical spending and key health care quality indicators.” 

The Baltimore-based insurer said annual medical costs were trending upwards of 7.5% annually when the PCMH launched in 2011. Annual increases have since been limited to 3% overall, with increases for patients within the PCMH at an “unprecedented” 2%.

From 2011 through 2015 year-to-date, CareFirst members have experienced, at a per 1,000 member rate:

  • 19% fewer hospital admissions

  • % fewer days in the hospital

  • % fewer hospital readmissions for all causes

  • % fewer outpatient health facility visits
